Credit Card on File and No-Show Policy

Credit Card on File Policy

At Salt Lake Dermatology and Aesthetics, we require all patients to keep an active credit card on file. We do not store this information in our system, rather it is stored securely through our payment processor Square, and we have no access to your credit card number or personal financial information.

No-Show and Cancellation Policy

Each time a patient misses an appointment without providing proper notice, another patient is prevented from receiving care. Therefore, patients who do not show up for their appointment without a call to cancel at least 24 hours before the appointment time will be considered as a “no-show” and may be charged a fee.

No-show fees will be billed to the patient. This fee is not covered by insurance and must be paid in full prior to scheduling your next appointment.
